The thing that upsets me is that me and my wife are private, paying a monthly fee to a practice that has no and seems unable to employ a dentist to do anything . If we cannot run a private surgery in Dorset how are we going to provide any nhs service. My OH is on Denplan, and has to badger the practice to perform the contractual obligations, the latest "scam" is they now charge £50 to clean the room if they do a filling or extraction. this is on top of the denplan fees

I would say "find another dentist", but apparently that isn't so easy to do as say. If I thought I was being scammed by my dentist, the last thing I'd want is trusting them with my dental care.

What's the difference if it's a filling, extraction or just a general check up?

I think I have a tooth related story that can trump anything mentioned so far.
Notwithstanding my P2P investments, I have plenty of cash stored away for a rainy day, and I am a big fan of long haul travel. There are lots of great business class flights out there, so long as you don't start your journey in UK. So in 2019 I was planning on spending December in South East Asia, with Christmas on the beach in Thailand. However, in order to get the cheapest flight I have to fly from Stockholm via Doha to Hong Kong with Qatar airways, and then Hong kong to Saigon with Cathay Pacific. I boarded the QR flight at ARN at about 4pm and after a couple of glasses of wine I tucked into my roast Lamb main course.
First mouthfull I felt a sharp pain in the top left side of my mouth. Managed to continue eating on the right hand side of my mouth, but it was obvious that something was wrong (toothache at 38,000ft aint fun). It wasn't too bad so long as I didn't chew on the left side, so I managed to get through Doha and onto Hong Kong, even though by this time I wasn't feeling well. I was fortunate to have access to the first class lounge in Hong Kong, but spend the entire 2hr transit time looking on the internet for an Expat dentist in Saigon. By the time I got there I was suffering from a serious dose of flu, as well as the toothache. Spent 2 days in the hotel in Saigon and decided to abandon my holiday and fly home.
When I got home getting an appointment with my dentist was easy (nearly as easy as her extracting my wisdom tooth), but it took 3 months to get my travel insurance to pay up for the cost of the curtailed holiday.

Have they added the charges since you took out your last denplan 'annual' contract ? I'd be going apes*** at this. During Covid my dentist, when they were allowed to restart treatments, did add additional fees on top of their prior normal rates (I wasn't on denplan). But as I recall it was at worst £20 to cover the cost of additional PPE and clean up between treatments. Which frankly seemed entirely reasonable. Fees they have since dropped.

It would not work for dental emergencies but I use, and have used for almost 10 years a dental surgeon in Fuengirola Spain for routine check ups and any treatment that the check up shows I need. The dentist is British trained but retired to Spain 12 years ago as he was fed up of the NHS. His partner at the surgery is from Norway and speaks better English than I speak Spanish. Check ups and fillings. crowns, scale and polish etc are also about two thirds of the price I would pay in the UK which makes my week in Spain in Autumn and Spring a cheap break. I win all round. I've never had a bill over £350 but normally it's between £140 - £250 twice a year - but I do try to look after my teeth.

Went on Friday for my checkup, and after the new dentist took x rays, she said I needed to see the hygienist, "unfortunately we no longer provide NHS hygienist services, its private the fee is £49, are you willing to pay"
No I'd had no prior notice, so felt rather cornered.
So now my checkups will cost me £63, £49 private hygienist, £14 for NHS checkup. It's not that different to the £49 NHS checkup and hygienis, but personally I fear that some will drop the hygienist to save the £49 especially given how tight money is for some.

This is odd because we've had Denplan for many years and understand it to be a direct contract between the patient and their specific dentist (and not with the practice). If that dentist has disappeared, it sounds like they've broken the terms of your contract and are now taking your monthly payments under false pretences. I would be contacting the Denplan organisation with a complaint and asking to move elsewhere.

My hygienist has been off on maternity leave. Her young replacement has been great, spent most of the appointments actually cleaning my teeth! And says how well I am looking after them. The 'proper' one is now due back, in the past she has spent half of the appointment checking this and that (feels like she's doing a thesis, lots of computer inputs) and then telling me how bad my teeth/gums have got since the last visit (my dentist says nothing has changed), she usually gets to actually cleaning my teeth well through the appointment and always seems to advise I should get more treatment done at a big expense obviously. I now hate (again) the idea of going back. Also looking for me to book a month early with payment upfront!
